Deferral Changes
Enclosed, please find a Voluntary Deferral Change Form. The Omaha/Lincoln area members may make changes at this time, effective March 1, 2012.
If you would like to make changes at this time, please complete the change form and return it to Benefit Plans, Inc. You may fax the form to 891-0444, Attn Martha or scan it through email to marthad@bpiomah.com . Your form must be received no later than Monday, February 27, 2012, in order to assure it’s processed for March 1, 2012.
After this date, there will be no changes allowed for Omaha/Lincoln area members until September 1, 2012, at which time you shall receive another form. There is one exception: you are allowed to change your deferral to $0.00 at any time. You can contact Martha Dean at Benefit Plans for a form if the need should arise (402-891-0808). (Sioux City members may not make changes at this time unless they are dropping to $0).
Please do not send requests for fund transfers with this form; BPI is not authorized to perform these transactions. Investment questions should be directed to Mark Slattery or Shannon Case at 402-502-5700.
*As a reminder, you do have 24 hour access to your account at Nationwide via the phone and internet. You can make changes to your future allocations and/or transfer money among funds.
You can call the Nationwide Inquire Line at 1-800-772-2182. First time users will be asked to enter their social security number and will need to use 1234 for their initial personal identification number (PIN).
You can also log on to your account at the Nationwide website: www.nationwide.com.
Participants will choose “401K Account” from the “Access My Accounts” drop down menu. First time users will then want to click on the link for “Register-first time using this site.” Participants will enter their social security number and case number where prompted. The case number/account number assigned to this plan is 253-80044. Click continue and follow the on-screen instructions to finish setting up access.
